The FDA regulates supplements as food — not as medicine. Manufacturers do not need to demonstrate efficacy. The claims on the label are largely unverified. And the industry is, to put it gently, not uniformly trustworthy.

This does not mean supplements are useless. It means you need to be selective, evidence-led, and appropriately skeptical. The good news is that several supplements have genuinely strong evidence behind them — particularly for women navigating the hormonal shifts of perimenopause and menopause.

This post covers the ones worth your time and money, the ones worth skipping, and the specific things to look for when choosing quality products. I have spent years reading the research on this. Here is what I have concluded.

First: The Principle That Changes Everything

Before specific recommendations, one principle worth internalizing: supplements are meant to supplement a good diet and lifestyle, not replace them.

The research consistently shows that isolated nutrients in supplement form rarely replicate the benefits of the same nutrients consumed in whole food form. The matrix of cofactors, fiber, and phytonutrients in whole foods creates effects that supplements cannot fully reproduce.

This matters because it sets the right expectation. Supplements fill genuine gaps and provide meaningful support. They do not compensate for a poor diet, inadequate sleep, chronic stress, or a sedentary lifestyle. The lifestyle foundations matter more.

With that caveat clearly stated — there are several supplements that have strong, consistent clinical evidence for meaningful benefit in women over 40.

The Supplements With Strong Evidence

Magnesium Glycinate

If you take only one supplement after reading this post, let it be magnesium glycinate.

Magnesium is involved in over 300 enzymatic processes in the body — including energy production, protein synthesis, muscle and nerve function, blood sugar regulation, and blood pressure control. It is also a cofactor in the production of GABA, the calming neurotransmitter that promotes relaxation and sleep.

Here is the critical point for women over 40: progesterone — which declines significantly during perimenopause — has a similar calming, GABA-supporting effect on the nervous system. As progesterone falls, many women experience increased anxiety, disrupted sleep, and a heightened stress response. Magnesium partly fills that gap.

Women over 40 show increased prevalence of deficiency due to decreased skin synthesis efficiency, reduced dietary intake, and the fact that vitamin D is fat-soluble, meaning absorption depends on adequate dietary fat.

The clinical evidence is substantial. The Women's Health Initiative study found that vitamin D3 supplementation with calcium reduced hip fracture risk by 12% in postmenopausal women. A 2020 meta-analysis showed that achieving adequate serum vitamin D levels improved muscle strength by 7 to 13% and reduced fall risk by 35%. Why K2 matters: vitamin D3 increases calcium absorption from the gut — which is beneficial for bone density. But without vitamin K2, that calcium can deposit in arterial walls rather than in bones. Omega-3 Fatty Acids (EPA and DHA)

The omega-3 fatty acids EPA and DHA are long-chain polyunsaturated fats that the body cannot produce efficiently on its own. They are essential for cardiovascular health, brain function, and the regulation of inflammation throughout the body.

For women over 40, the evidence is particularly compelling across several dimensions. A 2025 review published in Post Reproductive Health found that omega-3 fatty acids may specifically support brain health during the menopausal transition. Look specifically for the KSM-66 branded extract — it is the most extensively studied form of ashwagandha, with the largest number of published clinical trials and the most consistent quality control. What to Be Skeptical About

Biotin for Hair

Biotin deficiency is extremely rare, and supplementing above adequate levels does not produce meaningful benefits for hair growth or thickness in people who are not deficient. If you are experiencing significant hair thinning after 40, the far more likely causes are thyroid function, iron levels, hormonal changes, or protein intake — all worth investigating before spending money on biotin.

Most Collagen Creams

Collagen molecules are too large to penetrate the skin. The collagen in a cream cannot reach your dermis. Save the money for oral collagen peptides, which have actual evidence.

Very High Dose Vitamin C Supplements

Vitamin C is genuinely important for collagen synthesis and immune function. However, mega-dose vitamin C supplements — typically marketed at 1000mg or above — provide minimal additional benefit beyond what lower doses achieve, and at very high doses can cause digestive issues. Getting 250 to 500mg daily, either through diet or a modest supplement, is what the evidence supports.

Detox Supplements

Your liver and kidneys detoxify your blood continuously and efficiently. There is no clinical evidence that any supplement meaningfully supports or enhances this process in healthy individuals. The 'detox' supplement category is largely a marketing construct.

Since supplements are not regulated for efficacy, third-party testing becomes the most important quality indicator available.

The best starting point is to have a comprehensive blood panel done — vitamin D, iron, B12, thyroid, and inflammatory markers — before spending money on supplements. Supplement what is actually low. Food the rest.
